During the school year, many forms need to be filled out. In an effort to make it easier for you to access these forms easily, we have posted them here.

Pre-k Lottery and Out of Bound Lottery: On January 28, 2010 thru Feb 28, 2010, the Pre-k and out of bound lottery applications will become available on dcps.dc.gov. The lottery application must be completed online; no paper applications will be accepted. Requirements and information (pdf):

• Pre-school and pre-kindergarten
• Out of Boundary K-12

Photo Release Form: During the school year, parents and teachers take photos of students that are posted on the website, in the newsletter and the art blog. We need permission from you to post these photos. We do not name individual children in any photos.

Field Trip Form: Every grade goes on field trips throughout the year, these forms give your student permission to participate.

Lip Sync Forms: The Student Council hosts a lip sync competition that is photographed and filmed. Parental permission and photo release forms need to be filled out to participate. Soccer Field Use Forms: If you are interested in using the beautiful Murch Soccer Field for practice or games, fill out a field use application, sign a user agreement, read the field rules and your team is on its way to having a great practice venue! Volunteer Sign-up Form: Murch depends on its volunteers. If you have any time to volunteer during the school year please fill out the form and return it in your child's backpack.

Room Parent Form: Room Parents support the teacher and students by helping plan classroom events,communicate with parents, and coordinate activities for the class such as the Fall Fair booth, parent pot luck, art project for the auction and class gifts for the teacher. Support Our Schools Form: Every Fall, the Home School Association (HSA) raises funds to pay for additional staff positions at Murch including teacher aides and recess aides. This annual funding drive is key to Murch's success. We ask for donations to be contributed by October 31; but still are happy to take donations throughout the year. Please fill out the form and send back to school in your child's backpack.

Auction Donation Form: Every Spring, Murch organizes a gala Auction night. Its success depends on your contributions.
